Have this ready when you call
- Patient's name, date of birth and address
- Insurance cards (Medicare, Medicare Advantage, Medicaid or private)
- A short description of the foot problem
- A list of current medications
- The best phone number for a family member or caregiver
- Gate codes, building entry or facility check-in instructions
Getting ready for the visit
- A chair for the patient, with good lighting nearby
- The shoes and socks the patient wears most often
- Pets secured, if possible
- A family member or caregiver present, or available by phone
